Requirements for Notarization
Personal appearance of the affiant/signatory before the notary public
Valid government-issued ID with photo and signature (e.g., passport, driver's license, PRC ID)
The document must be complete and not contain blank spaces
The notary must record the notarial act in the Notarial Register with the entry number, date, and type of act
Notarial Register
Every notary public must maintain a Notarial Register containing a chronological record of all notarial acts.
The register must be preserved for at least 10 years from the date of the last entry.
Failure to maintain the register is a ground for disqualification as a notary public.
